Job summary

The post holder will work as part of a dynamic team, includingcommissioners and providers in delivering an effective servicesupporting people with Learning Disabilities in their CHC Journey

The role involves; assessing eligibility criteria under the NHS National Framework for Continuing Healthcare and Funded Nursing Care 2009 (2022), planning and commissioning packages of care (including Personal Health Budgets), supporting individuals, their families and health professionals through the Continuing Healthcare process (including transition from child to adult services) and reviewing eligibility / packages of care.

Main duties of the job

The post holder will undertake comprehensive professional Continuing Healthcare and Funded Nurse Care assessments for adults over 18 years of age with complex health needs including the assessment, review and monitoring of care for those whose health needs are funded via NHSContinuing Healthcare within the dedicated time scales. This will include retrospective and appeal cases.

Job responsibilities

The post holder will work jointly with health, social care and independent sector colleagues to ensure care management processes are appropriate and at time may be asked to present assessments to the BSOL continuing care panels for ratification.
To follow the process laid out in the National Framework for NHS Continuing Healthcare; to include organisation, co-ordination and participation in MDT eligibility recommendations to be verified by ICB decision makers.
The post holder will be responsible for complex clinical caseload comprising of; Assessments, undertaking of review (3 / 12 CHC review unscheduled review and Fast Tracks and implementation on Personal Health Budgets and ongoing Case Management of Patients fundedby Continuing Healthcare (CHC), Mental Health & Learning Disabilities.
Ensure assessments conducted are robust, transparent and multi-agency (where required) for NHS Continuing Healthcare / NHS-funded Nursing Care.
To support the seamless implementation of the National Framework for Continuing Healthcare including contribution to the development of training programmes and delivery plans as required to facilitate others learning.
Within Birmingham and Solihull ICB provide advice, screen and coordinate assessments against the National Framework for Continuing Healthcare and Funded Nursing Care within Nursing Homes.
To work with the Senior Nurses to process Funded Nursing Care and Continuing NHS Health Care Applications. Influence best practice by providing professional nursing advice on a range of physical and mental health nursing issues, which will require the individual to work in collaboration with partners and other professionals to ensure patients' health care needs are met appropriately and effectively.
To work closely with the Funded Nursing Care Lead Nurse by assisting in observing the quality of care in homes visited, identifying unmet need and being proactive in the encouragement of improvement in care standards.
A strong business focus as well as clinical expertise is a key requirement for this role to ensure delivery of best possible care within available funding representing "value for money".
Assist in the implementation of policies involving National Framework for Continuing Healthcare and Funded Nursing Care in Care Homes with Nursing.
To assess new applications for clients eligibility against the National Framework for Continuing Healthcare and Funded Nursing Care
To co-ordinate new applications for consideration of eligibility for Continuing NHS Care
To undertake Nursing Assessments for patients with identified health needs.
To complete the Decision Support Tool and recommendation as indicated.
To complete Continuing Healthcare Review assessments and complete appeal assessments.
To provide Clinical Cover to include processing Fast Track applications; preparation of cases for panel; and provide advice and guidance to external agencies.
To make recommendations following Nursing Assessment relating to all identified needs of a client

Report any concerns related to client care following assessment to the Senior Nurse within the Continuing Care Team, Care Quality Commission, Adult Protection and Social Services
To participate in Safeguarding of Vulnerable Adults process related to individuals in Care Homes with Nursing.

To identify specialist nursing equipment needs and advise about provision of that which is over and above the minimum standard.
To ensure suitability of equipment for patients with identified health problems
To ensure every assessment (new or review) represents value for money within available funding.
To work within agreed systems and processes to secure best value for money packages that meet clinical needs

Significant experience of working patients with complex and long-term health needs, disabilities and those with life limited / palliative or end of life

Educated to degree level or equivalent level of experience working in a specialist area

Conversant with national and local guidance on NHS Continuing Health care / NHS Funded Care for Adults Continuing Healthcare and with detailed understanding of the correct use of the national tools (checklist, DST, Fast Track)

Desirable

Experience in community or acute nursing desirable
