Wakefield are seeking an experienced children's nurse to work alongside the team leader in the children's continuing care service. As an experienced member of this progressive, friendly team you will assess and case manage the health care needs of children and young people with complex physical health needs. High level of assessment skill is required in order to establish eligibility in order to provide an appropriate level of support to the child and family. Currently using the NHS National Framework for Children and Young Peoples Continuing Care 2016, to determine this. The work base is White Rose House in Wakefield, currently remote working is employed, with face to face work with children and families and also partner agencies. We encourage multi agency working, so that holistic care is provided. Good communication skills are required for this role and the ability to work as part of a team and to be able to manage a caseload of patients effectively. In your role, alongside excellent assessment and co-ordination skills we are looking for individuals with a strong commitment to partnership working in order to ensure our clients receive full care provision according to individual needs. Each package varies according to need and delivery of health care and we are looking for a Nurse Assessor who is able to able to think with and support families to achieve their own individual positive health outcomes.
Main duties of the job
Collating and undertaking comprehensive and coordinated assessments for decision making for Continuing health Care with children and families within health, social and education care.
Promoting Personal Health Budgets, ensuring integration into Childrens continuing care process.
Attending decision making panels to present own cases and provide clinical information.
Case managing children eligible for continuing care, ensuring timely reviews are conducted and packages continue to be effectively commissioned and are cost effective;
Developing strong working relationships with partner agencies including Acute Hospital Trusts, local authorities, schools, the Independent Sector and others.
Have a forward thinking approach.
Oversee the service in the absence of the team leader.

Possible secondment opportunity.
Lead and Manage Multi-disciplinary team ( MDT) meetings as appropriate to support completion of the Childrens continuing care assessment process and make eligibility recommendations for continuing care in accordance with the National Framework for Childrens continuing care.
Complete Decision Support Tool (DST) Assessments or other assessments as appropriate to role and referral.
Visiting children and families in their homes, and other settings (hospitals, residential provision, schools etc.) as part of the assessment process.
Be proactive in the transition process for young people being assessed for adult continuing healthcare.
Promote and support implementation of Personal Health Budgets (PHB) to children and young people as appropriate.
Be involved in the regional children's continuing care forum meetings.
