Duties and Responsibilities :

Conduct medication reviews and optimise prescribing practices within GP practices.
Work collaboratively with GPs and other healthcare professionals to improve patient care.
Provide expert clinical advice on medication use and safety.
Support chronic disease management and deliver patient education to enhance health outcomes.

Qualifications :

Registered with the General Pharmaceutical Council (GPhC).

Independent Prescriber qualification is highly desirable.

Strong knowledge of clinical guidelines and prescribing best practices.

Excellent communication and organisational skills.
