• Weekday : £27 per hour
• Nights & Saturdays : £32.85 per hour
• Sundays & Bank Holidays : £40.82 per hourContract Type : Full-time, flexible shifts availableRole Overview : We are seeking a skilled Community Nurse to join our dedicated healthcare team in Barnstaple. The successful candidate will deliver high-quality care in patients’ homes, supporting those with chronic illnesses, disabilities, or recovering post-hospital discharge.Key Responsibilities :
• Provide comprehensive nursing care to patients in their own homes or community settings.
• Administer medications, wound care, and health monitoring services.
• Coordinate care plans with other healthcare professionals.
• Educate patients and their families on managing health conditions.
• Support hospital discharge processes and reduce hospital readmissions.Requirements :
• Active NMC registration.
• Proven experience in community or district nursing.
• Strong organizational and time-management skills.
• Ability to work independently and make sound clinical decisions.
• Flexibility to travel within the assigned area.Desirable Skills :
• Advanced clinical assessment skills.
• Experience working with patients who have long-term health conditions.
