Are you an experienced Band 3 / Band4 Therapeutic Assistant Practitioner seeking a new and diverse challenge?

West Lothian continues to invest in delivering community assessment and rehabilitation closer to home through our Home First programme. This programme has seen significant investment in our community AHP teams and has recently secured funding to introduce a community single point of contact to enhance the patient pathways.

We are looking for experienced Band 3 / Band 4 Therapy Assistant Practitioner to strengthen our growing team and deliver person-centered assessment and therapy across our pre-admission community pathways, discharge to assess function, virtual community hospital ward, and within our step-down Intermediate care facilities.

The successful applicant would be joining a very stable and dynamic multi- disciplinary team with a passion for integrated working putting patient outcomes and experience at the heart of their practice. Including working closely with our inpatient colleagues to identify and facilitate early supported discharge and provide rehabilitation in the community.
