The Directed Enhanced Service contract for PCNs provides new and exciting opportunities for experienced physiotherapists to work in a new environment and in different ways.
Main duties of the job
First Contact Physiotherapists operate at an advanced level of clinical practice, with skills to assess, diagnose, treat and manage musculoskeletal (MSK) problems and undifferentiated conditions. This will involve seeing patients, without prior referral from their GP, to establish a rapid and accurate diagnosis and management plan, thus streamlining pathways of care. They work independently in clinical practice and do not require day to day supervision. Patients can either self-refer or be referred by the networks members.
The post requires experienced a First Contact Physiotherapist with a passion to improve patient health outcomes and improve efficiency of the Primary Care network by providing direct, accessible, and timely musculoskeletal expertise face to face with patients. The post holder will proactively transfer workload relating to musculoskeletal issues to improve patient care, safety and free up all clinical staff to spend more time on clinical care.

Duties and Responsibilities
- Build relationships with key GP Practice staff within the PCN and participate in all treatment and preventative healthcare services as delegated and agreed by the Operations Director.
- Assess, diagnose, triage and manage patients, taking responsibility for the management of a complex case load.
- Receive patients who self-refer (where systems permit) or are referred from a clinical professional within the PCN, and where required refer to other health professionals within the PCN
- Progress and request clinical investigations (such as x-rays and blood tests) and referrals to facilitate diagnosis and choice of treatment regime, understanding the limitations of investigations
- Develop integrated and tailored care programmes in partnership with patients, providing a range of first line treatment options including self-management, referral to rehabilitation focussed services and social prescribing.
- Manage complex integrations, including working with patients with psychosocial and mental health needs, referring onwards as required and including social prescribing when appropriate.
- Establish programmes to facilitate behavioural change, optimise patients physical activity and mobility, support fulfilment of personal goals and independence and reduce the need for pharmacological interventions
- Work across the multi-disciplinary team to develop and evaluate more effective and streamlined clinical pathways and services
- As part of case management, liaise with secondary care musculoskeletal services, community care musculoskeletal services and local social and community interventions as required.
- Take responsibility for using professional judgement to make and justify decisions in unpredictable situations, including in the context of incomplete or contradictory information.
- Communicate effectively and appropriately with patients and carers, complex and sensitive information regarding diagnosis, pathology, prognosis and treatment choices supporting personalised care
- Implement all aspects of effective clinical governance for own practice, including undertaking regular audit and evaluation, supervision and training
- Regularly review latest guidance ensuring the practice conforms to NICE, CQC etc.
- Encourage collaborative working across the health care and be a key contributor to the primary care networks providing leadership and support on musculoskeletal clinical and service development across the PCN.
- Support the delivery of enhanced services and other service requirements on behalf of the PCN
- Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through clinical incidents and near-miss events
- Support regional and national research and audit programmes to evaluate and improve the effectiveness of the First Contact Physiotherapist (FCP) programme. This will include communicating outcomes and integrating findings into own and wider service practice and pathway development.
- HCPC registration, supported by a professional culture of peer networking, review and engagement in evidence-based practice
- Completion of the FCP roadmap
- Undertake continual personal and professional development, taking an active part in reviewing and developing the roles and responsibilities
- Provide learning opportunities for the whole multi-professional team within practices throughout the PCN.
