Are you an experienced RMN looking to make a meaningful difference in dementia care? We are seeking a Registered Mental Health Nurse (Band 6) to join our client's multi-disciplinary Memory Assessment Service (MAS) team in Morecambe. This is a fantastic opportunity to work in a specialized role, supporting patients and their carers through high-quality assessment, planning, and care.
Key Responsibilities :
- Serve as an independent specialist practitioner, ensuring effective assessment, planning, consultation, and formulation towards diagnosis within the Memory Assessment Service.
- Demonstrate specialist knowledge and understanding of strategies and interventions for dementia treatment.
- Proficiently refer patients to other agencies within the service or external services as necessary.
- Confidently identify risk factors and take appropriate actions to mitigate them.
- Facilitate clear and effective communication between GPs, LCFT services, and non-statutory services to uphold the highest standards of patient care.
- Provide mentoring and clinical supervision to staff and trainees on placement.
- Actively promote Trust values within the MAS service, aligning with NMC and HPC codes of conduct.
- Support the team manager in providing leadership within the MAS team, ensuring robust assessment, formulation, planning, and monitoring of care for patients and their carers.
Pay Rates :
Days : £22 - £26 per hour
Nights / Saturdays : £30 - £34 per hour
Sundays / Bank Holidays : £38 - £44 per hour
Requirements :
