The GP Health Referral Facilitator is an important role at Bodmin Leisure Centre to help us deliver our health and wellbeing objectives in our Cornwall Partnership. The GP Health Referral Facilitator will be GLL’s key contact to link in with GPs and Health Professionals in the borough as well as working alongside the local authority. Working in a small team the GP Health Referral Facilitator will manage and oversee successful delivery of the Healthwise programme.

What you’ll do :

• Manage the Healthwise Programme and Healthwise team.
• Be the key contact for GPs / Health Professionals and the local authority
• Complete practical as well as administrative elements of Healthwise
• Represent Healthwise in internal and external meetings.
• Produce monthly, quarterly and annual reports.
