Position :
An opportunity has arisen for a Project Support Facilitator based at Brodick Pharmacy on the Isle of Arran on a fixed term / secondment opportunity for 3 years.
What you will do :
NHS Ayrshire & Arran is working in partnership with Macmillan Cancer Support to launch a new service across Ayrshire and Arran called ‘Improving the Cancer Journey’ (ICJ). We have an exciting opportunity for a highly motivated and enthusiastic individual to support people affected by cancer on Arran.
In partnership with Macmillan Cancer Support, the purpose of this post is to provide a person-centred service that enables people affected by cancer to navigate the health, social care and wider systems to access practical, emotional and social support and promote self-management for people to live well, addressing their holistic needs during and after cancer treatment. There will be a requirement to work across organisational boundaries with partners to ensure the best possible outcomes for people affected by cancer.
You will work in a ‘Macmillan Professional’ branded role and duties will include meeting with Arran residents who have received a cancer diagnosis in order to carry out a Macmillan Holistic Needs Assessment. Together, you will develop an Individual Care Plan tailored to their specific needs. These meetings will take place across a variety of locations and settings. You will offer advice, build capacity and support the development of the ICJ service by working in partnership and signposting and referring to appropriate partner agencies and services.
The Arran post will be employed by NHS Ayrshire & Arran within North Ayrshire HSCP and seconded to work within Arran Cancer Support Trust (ArCaS). The admin base for the role is the ArCaS Hub within Arran Pharmacy Brodick. There is a requirement to work flexibly across Arran to meet the service demands.

Knowledge, training and / or experience required to do the job :
Ideally, you should be educated to HND / SVQ (Scottish Vocational Qualifications) level 4 in health and social care, community development and / or equivalent relevant health and social care experience. You will have experience of delivering a
person-centred service in a support / advice giving role and have the ability to demonstrate a proactive approach to problem solving and decision making. Additionally, you will have well-developed communication and interpersonal skills with the ability to deal with complex and emotional situations.

You will be equipped to work remotely, with regular travel within Arran. On occasion, there may be a requirement to travel to the mainland for training.
