Job description

Job summary

Retford & Villages Primary Care Network are seeking a driven, emotionally intelligent, high quality ANP who values excellent patient care; to see patients, working across all practices, seeing patients on acute presentation.

The successful candidate will act as an autonomous practitioner working independently, to provide complete episodes of care for patients of any age, management of a wide variety and range of presenting complaints and health care needs. To assess, diagnose and treat conditions of patients attending primary care, initiating direct referrals as appropriate.

You will provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.

To develop new and innovative concepts, models, methods and practices to deliver and improve primary care services in order to meet the needs of the organisation.

Main duties of the job

Undertakes clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services.

Assesses diagnoses and treats patients attending primary health care services with a range of acute, non-acute and chronic medical conditions.

Directly admits patients to secondary care with acute medical needs.

Refers patients to other medical specialities for assessment if deemed appropriate.

Refers patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for x-ray.

Interprets a range of diagnostic tests and routine clinical procedures.

Makes critical clinical judgements to satisfy the expectations and demands of the job for example, first contact with patients presenting acutely and manages care in the presence and absence of protocols.

Makes decisions where appropriate without recourse to others direct referral of patients to hospital consultants without prior reference to other medical colleagues.

Works within professional guidelines and codes of conduct.

Retford& Villages PCN comprises of 5 General Practices : Riverside Health Centre,Kingfisher Family Practice, Crown House Surgery, Tuxford Medical Centre andNorth Leverton Surgery. With a population of over 57,000 people, we are largestof the three PCNs in Bassetlaw, responsible for 44% of the registered patients.

Thearea we cover is largely rural. Although the area served is relativelyaffluent, there are more deprived families and communities clustered around thetown of Retford. A higher percentage of our patients are supported inresidential or nursing care than the national rate.

Person Specification

Experience

Essential

  • Understanding of computer based medical records management SystmOne / EMIS
  • Significant post registration experience.
  • Recent primary and community nursing experience.
  • Nurse-led management of minor illness, minor ailments, and injuries.

Desirable

  • Worked in a General Practice Environment
  • Working with community development initiatives.
  • Health-needs assessment.
  • Compiling protocols and clinical guidelines.
  • Audit.
  • Research.
  • Knowledge and Skills

  • Insight and understanding of current issues in nursing and NHS
