Main duties of the job

We have an exciting new position within one of our Primary CareNetworks working as a social prescriber. This person will have experience or apassion for working with people living with complex and sometimes challengingissues. You will work with the person to help them identify what really mattersto them and to support them in working towards achieving their goals byconnecting them to support in their community. Helping to improve health andwellbeing, reduce demand on services and addressing health inequalities acrossthe city.

As an advocate for patients and an ambassador forYork CVS, you will not be afraid to challenge in the best interests of thepeople you are supporting.

This post will require some weekend and evening work.

Job responsibilities

To be part of the city-wide social prescribing initiativeacross York based in the community and managed by York CVS. This a dynamic andforward thinking team advocating for patients whilst working in closepartnership with health and social care to respond to the pressures within thehealth and care system.

Link Workers provideone-to-one support to clients to identify what matters to them and offerinformation, advice and support to help them to :

  • Explore options to addressnon-clinical needs
  • Improve their long-termhealth and wellbeing
  • Address health inequalities

This involves working in amulti-disciplinary environment and developing networks between the voluntaryand community, health and social care, and primary care sectors.

As a Social Prescriber, youwill proactively work with individuals, getting to know the person, developinga positive relationship with them, understanding the challenges they face andthe changes they want to make. You will workalongside patients to create personalised care support plans facilitatingempowerment and independence.

Referralsto this service can be complex and will require empathy and innovativethinking, excellent listening skills and the ability to build strongrelationships whilst maintaining professional boundaries.

You will work collaborativelywith local VCSE, health and social care, and primary and secondary careorganisations, to support people with their health and wellbeing needs.

Establish links and buildworking relationships with primary and secondary care colleagues and otherorganisations to identify, build on, and support a range of services andcommunity activities to promote health and wellbeing.

Build and maintain knowledge onlocal provision to help promote to clients and others.

Duties and Responsibilities

To provide support to people ina planned and structured way, co-producing personalised care plans to helpimprove their health and well-being.

To make best use of the localVCSE sector, connecting and re-connecting people to community resources

To improve peoples healthliteracy, helping them to be better equipped to manage their (often multiple)long-term health conditions.

To develop good workingrelationships with referral partners to develop their understanding of theimpact social prescribing can have, and to identify people who would benefitfrom the service.

To keep accurate clientrecords, collate qualitative and quantitative data and feed in to quarterly andannual reporting.

To forge strong links with theVCSE sector; identify gaps in provision locally and collaborate with VCSE partnersto help address those gaps.

Build and maintain good workingrelationships with health and care professionals and VCSE partners across York.

To take responsibility forspecific projects, developments, and initiatives. This may include working witha range of clients and in different locations.

To be an ambassador for YorkCVS and social prescribing.

  • Experience

    Essential

  • Significant experience of working with vulnerable people who may have multiple needs and complex lives.
  • Experience of person-centred service delivery in a community or clinical setting advocacy, support work, counselling or social work.
  • Experience of working collaboratively and in partnership with statutory and VCSE sector organisations.
  • The ability to work autonomously and to plan, prioritise work under pressure and adapt to new models of working.
  • Extensive experience of using the Microsoft Office suite.

  • Relevant qualification in Community Development, Health or Social Care, to degree level or equivalent experience
