Job descriptionProvide comprehensive nursing care to patients in their own homes or community settings. Administer medications, wound care, and health monitoring services. Coordinate care plans with other healthcare professionals. Educate patients and their families on managing health conditions. Support hospital discharge processes and reduce hospital readmissions. Active NMC registration. Proven experience in community or district nursing. Strong organizational and time-management skills. Ability to work independently and make sound clinical decisions. Flexibility to travel within the assigned area. Advanced clinical assessment skills. Experience working with patients who have long-term health conditions. Knowledge of palliative and end-of-life care.

- Weekday : £27 per hour
- Nights & Saturdays : £32.85 per hour
- Sundays & Bank Holidays : £40.82 per hour
