Job summary

A new team is being established to work alongside the within the Unplanned Care Response Team for the West Place and to support insulin administration / diabetes care in the community. The applicant should ideally have excellent communication skills with a good understanding of IT systems. Previous Community Nursing experience would be beneficial for this position.

We are looking for a well-motivated team player who will be expected to work in a busy environment and able to meet the demands of this hard-working service. Support and any additional training would be provided by the teams and Clinical Education Team to ensure competency and skills are established and maintained.

Main duties of the job

Key Areas of Responsibility

1. To work within the integrated team to facilitate early discharge from hospital2. To work within the integrated team to prevent unnecessary admission to hospital3. To work with all health care professionals, and statutory / non statutory agencies to provide a seamless, integrated service to our service To work collaboratively with the Band 5 Peripatetic Insulin Nurse to ensure caseload is To support West Unplanned Care Response Team with patient visits such as urgent INR's, ecgs, wound care and double-up To work closely with Triage and Diabetes Specialist Team to ensure new patient discharges are reviewed in the community To support patients with re-enablement following Hospital Admission or new diagnosis, wherever To work with all health care professionals, and statutory / non statutory agencies to provide a seamless, insulin administration to our service users.

This is a new team that will be based within our expanding Triage team, we have already established a new Unplanned Care Response service and are looking to increase further to include the Peripatetic Insulin Team. We have a wealth of experience and knowledge and are led by an experienced Clinical Operations Manager.

We provide excellent evidence-based care to patients, ensuring that safe, relevant and timely care is provided by the community nursing teams to the local housebound population of West Norfolk. We work closely with our GP and Social Services colleagues and visit patients in their own homes, care homes and clinics as appropriate.

Job responsibilities

The post holder will : 1. Apply competency-based treatment techniques / care to patients with specific conditions under the supervision of a qualified practitioner, following a prescribed Plan and prioritise own visits to delegated patients and ensure these are documented in personal work Ensure patient held records are completed for each visit and that SystmOne inputting is completed Make basic judgements on patients response to prescribed treatment / care plans, and report findings to a qualified Liaise with other healthcare professionals as Communicate with patients / carers by exchanging factual information, reassurance, tact and Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care Demonstrate dexterity and coordination when undertaking treatment of patients where accuracy is important, taking blood sugars, administering insulins, urgent INRs, ecgs, wound care, supervising, Be responsible for ensuring equipment is used safely, following relevant training and that it is maintained Contribute to the requisitioning of supplies and equipment through electronic ordering systems, and restocking of Work with qualified practitioners with group work / clinics as Demonstrate an awareness and understanding of consent and gain consent as appropriate following organisational Share responsibility of indirect patient contact tasks such as answering telephones, arranging appointments, processing referrals and inputting activity Undertake health promotion / education as required, smoking cessation, falls Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.
