Tameside Memory Clinic & Whittaker is a Screening Clinic & Day unit that provides Assessment & Diagnosis of potential Organic Illness as well as Community treatment and recovery sessions within Older People's Services for Tameside & Glossop which is situated in the Etherow Building at Tameside Hospital.

An opportunity has arisen in Older Peoples Mental Health Services at Tameside General Hospital for an enthusiastic, self-motivated Band 6 MH Practitioner.

The post holder will function as part of the MDT to assess & deliver the interventions required to support individuals through assessment and diagnosis and within the day unit offer a recovery pathway and equip them with the skills and confidence to enable a safe discharge from the service.

The post holder will be highly skilled and competent in conducting MH and diagnosis assessments, supporting through diagnosis, and developing outcome-based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence-based outcome measures.

Main Duties of the Job

Key Responsibilities of the Post :

  • To be responsible for the assessment, treatment, and evaluation of patients referred specifically for assessment, diagnosis & interventions.
  • To be able to work across all functions within the Memory service and day unit and support patients to achieve their individually agreed goals.
  • To work as part of a multi-disciplinary team to assess, plan, and implement individualised programmes of care / interventions.
  • To assess MH need and plan therapeutic interventions.
  • To lead on the development of group-based interventions to aid recovery and promote social inclusion.
  • To facilitate the development of peer support networks as part of the discharge pathway from the service.
  • To deliver a range of therapeutic interventions in line with professional requirements and in accordance with the job specification.
  • To support and facilitate Carer Support sessions.
  • To use a comprehensive range of verbal and non-verbal communication tools to communicate effectively with patients, their carers, and families and colleagues.
  • To be responsible for managing a caseload of clients as required.
  • To support patient discharge in a safe and timely manner, ensuring liaison with relevant professionals, to ensure that family and carers are, where appropriate, advised of the patients discharge arrangements, with timely completion of documentation.

Job Responsibilities

Key Requirements of the Person Specification :

  • We require an experienced person who is committed to the care of older people with Mental Health needs.
  • RMN, OT or equivalent training.
  • Substantial post-qualification experience.
  • Ability to provide leadership in the absence of the team manager.
  • Knowledge of MH legislation and guidance.
  • Knowledge and experience of working in an MDT.
  • Ability to manage and identify risks.
  • Skills to provide and receive clinical supervision.

    Education / Qualifications

    Essential

  • Registered Mental Health Nurse (RMHN) with current registration, Social Worker Qualification / OT
  • Completion of Preparation for Mentorship / Practice Teacher / Fieldwork Educator qualification or willingness to complete
  • Short Courses and experience to post-graduate diploma level.
  • Desirable

  • Registered General Nurse / Qualification related to physical health
  • Educated to degree level or working towards.
  • Non-medical prescribing qualification
  • Formal teaching qualification.
  • Experience

    Essential

  • Substantial experience as a mental health practitioner at Band 5 or above.
  • Experience of multidisciplinary working in mental health care with complex cases.
  • Experience of working to objectives and meeting deadlines, within a performance management framework.
  • Experience of supervising junior staff.
  • Experience of joint working with outside agencies.
  • Desirable

  • Experience of working in community settings.
  • Experience of managing referrals.
  • Knowledge

    Essential

  • Working knowledge of current mental health legislation and guidance in relation to Safeguarding and Vulnerable Adults.
  • Working knowledge of NICE guidance in the treatment of mental health problems.
  • Knowledge of mental health / risk management processes.
  • Be able to demonstrate an excellent understanding of clients presenting with physical and mental health comorbidity issues.
  • Overview and knowledge of the range of interventions and services that meet the needs of all service users referred to the mental health service including those with dementia, delirium, functional illnesses, self-harm, and substance misuse.
  • Be competent in developing and delivering training packages.
  • Display a good understanding of clinical governance and quality improvement.
