The Oncology, Palliative and Bereavement Service forms part of the ULTH Children’s Community Service.

Emotional / psychosocial interventions for children, young people and families who are affected by oncology diagnosis / palliative diagnosis and / or bereavement.

We are looking for a dynamic / creative individual to join our team. The post holder will be responsible for caseload management working with CYP, family / carers' / professionals- ensuring quality / excellence is given to all.

The service is Monday-Friday 9am-5pm-working flexibly according to the needs of individual children / young people / families, providing out of hours visits / occasional weekend work.

Main duties of the job

This challenging post requires a highly skilled, self-motivated, innovative individual experienced in undertaking comprehensive family assessments to enable the team to provide appropriate individual / direct family interventions. This role is emotionally demanding and in return for your commitment we offer regular supervision, a supportive team and CPD.

  • Support the Team Leader with the Daily Operational Management of the Specialist Family Practitioner Team.
  • In conjunction with the Team Lead set, maintain and provide high standards of working practices within the Specialist Family Practitioners Team.
  • Safeguarding children, young people and vulnerable adults (eg Parents / Carers).
  • To establish and maintain effective channels of communication with individuals, groups and all levels of the organisation and beyond, in order to provide an effective service.
  • To be an expert resource for professionals to enhance the services they provide for children with medical / nursing needs and their families.
  • To provide evidence based teaching / training to children with health care needs, their families / carers, and other learners, professionals in the wider community.
  • Professional development of self and contribute to the professional development and support of others.
  • Applicants must demonstrate an ability to work autonomously and as part of a team. The ability to travel throughout the county is essential. ULHT offers a lease car scheme for employees.

    Qualifications

  • Evidence of continual professional development and training.
  • Advanced communication skills course or willingness to undertake.
  • Teaching qualification or equivalent experience.
  • Evidence of on-going Safeguarding Children and Vulnerable Adults Training.
  • Relevant child related professional qualification in Health (NMC Registered Children’s Nurse, Health Visitor).
  • Specialist Child Bereavement training eg Post graduate Certificate in Childhood Bereavement.
  • Counselling Certificate or Diploma.
  • Previous Experience

  • Experience of working in a community role.
  • Experience of independent Caseload Management.
  • Evidence of Multi-Disciplinary Team and Interagency Working.
  • Evidence of working with children and young people with additional needs / complex disabilities including those who have life-limiting and / or progressive illness, critically / terminally ill.
  • Experience of supervision.
  • High levels of motivation & self-confidence.
  • Ability to work autonomously & as part of a team.
  • Proven written and oral communication skills.
  • Excellent Interpersonal skills.
  • Basic computer / keyboard skills.
  • Computer Literacy.
  • Experience of undertaking clinical audit and / or research.
  • Political awareness and knowledge / interest of current issues and interest and commitment to developing Children's Community Services.
  • Specific Requirements

  • Ability to work flexibly depending on the demands of the service.
  • Ability to travel across the county to community meetings and regional meetings as required.
  • Ability to travel throughout Lincolnshire as and when required.
