We are looking to recruit an enthusiastic, friendly and experiencedclinician to join our team. You will need to possess recognised qualificationsand experience to deliver autonomous care for our patients. To includeassessment, diagnosis and treatment to deliver quality patient care relating tominor illness on a daily basis.
The successful applicant will be taking clinical responsibility forpatients requesting an urgent appointment . As a team player you will beworking with and supported by, GPs, PNs and HCAs.

The practice has a list size of approximately 5,500 patients. Ourpractice team consists of 2 GP partners, 3 salaried GPs, and a well establishednursing team, with and an experienced admin and management team. All thedepartments and teams work together for delivering a safe and excellent serviceto our patients. We are set in the town of Poole very close to Poole Park andthe waterfront. We are part of Poole Central PCN and served by UniversityHospitals Dorset, including Poole, Bournemouth and Dorchester.

Job descriptionAssess, diagnose, plan implement and evaluate treatment / interventions and care for patients presenting with an undifferentiated diagnosis.Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.Prioritise work when working on duty triage and manage time accordingly.Expectations to always deal with medical emergencies within the practice.Support the nursing team when needed by reviewing cases, and advising on treatment.Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition.Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.Review and follow up patients as required to facilitate a good standard of care.Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.Recognise, assess and treat patients suffering with mental health conditions, follow referral process if necessary.Work within own scope of expertise and ask for help and advice when needed.Attend daily clinical huddle for reflection and feedback.Complete regular audits.Teach and mentor learners where appropriate. Be involved in team / practice learning sessions.
