Job descriptionConduct comprehensive assessments to determine individuals’ needs in accordance with the principles of the Social Services and Wellbeing Act. Develop and implement care plans in collaboration with multidisciplinary teams. Facilitate access to community resources. Conduct capacity assessments and address best interests. Advocate for individuals’ rights and services, ensuring ethical and legal compliance.
