An exciting opportunity has arisen for a fixed term contract to join our highly motivated Intermediate Care Therapy Team based within the Older People’s community Mental Health Team integrated with our health partners (EPUT). The post holder will provide a high-quality service to older people who require ongoing mental health support in the community. This is a key role in ensuring the council delivers high quality social care services to Thurrock citizens who are requiring mental health support and intervention
The successful candidate will be highly motivated and good at communicating. The role will include working within a multi-disciplinary team with an emphasis on reablement and a belief in the personalisation agenda and keen to work in an environment that puts its service users first. The team provides care social care, within the older people mental health services, working closely with health colleagues to avoid hospital admission and to enable early discharge.
A Criminal Records Bureau Disclosure at Enhanced level will be required for this post.
