Job Title : Salaried GPwER Dermatology

Salary : Dependent on experience

Hours : Sessions between 8 : 00am – 6 : 30pm, Monday - Friday). Potential overtime (including weekends).

Responsible to : Clinical Lead and Service Manager

Key Relationships : 32 General Practices, 4 PCNs, NWL ICB, Local Minor Surgery Services, Acute Specialist Service Departments (Level 4), Specialist Skin Cancer services, Secondary care specialties such as plastic surgery, rheumatology and diabetes.

BACKGROUND

Healthcare Central London Ltd (HCL) is the GP Federation owned by our 32 General Practices covering the Central London (Westminster) area. The organisation supports 4 Primary Care Networks (PCNs). We operate several NHS contracts on behalf of our PCNs including a Community Dermatology Service; Community Cardiology Service and an out-of-hospitals provision which is sub-contracted to our 32 General Practices.

On behalf of our practices we are the host employers of a large, and rapidly growing team of ARRS (Additional Roles Reimbursement scheme) roles including Clinical Pharmacists; Pharmacy Technicians; First Contact Physiotherapists; Dieticians; Paramedics; Social Prescribers; Care Coordinators, Digital & Transformation Leads, Nursing Associates, GPAs and Care Co-ordinators.

Community Dermatology Service

Healthcare Central London’s Community Dermatology Service (CDS) offers non-emergency care close to home for patients with a range of chronic skin conditions, including eczema, extensive acne and psoriasis. The service is available to any patient aged 16+ who is registered with one of HCL’s member Practices.

The overarching aim of CDS is to provide an innovative, high quality, consultant-supported GPwER service to Westminster patients. The service aims to be accessible and efficient, whilst managing the majority of diagnostics and outpatient activity within primary and GPwER care settings, and significantly enhancing the capability of GP’s and patients to effectively manage dermatological conditions.

ROLE PURPOSE

Due to expansion of our services, we are seeking a dedicated and experienced GP with an Extended Role (GPwER) in Dermatology, to join our dynamic clinical team our CDS clinics. The GPwER will play a key role in delivering high-quality community services to our patients and collaborating with other healthcare professionals to promote patient well-being.

Main duties of the job

As part of our clinical team, you will be committed to maintaining clinical continuity for patients in crucial circumstances. You will actively participate in learning events, audits, and other agreed-upon projects. We value teamwork and expect you to respect agreed ways of working, including prescribing practices. Your willingness to understand and improve practice systems, learn new IT systems, and support continuity of care will be highly regarded. Taking responsibility for your tasks and completing them efficiently will contribute to minimising unnecessary follow-up work. Collaboration with colleagues, mutual learning, and respect for multidisciplinary teams are essential. As an advocate for patient care, you will work across organisational boundaries when required. Staying updated with new guidance and addressing any time-management challenges will ensure smooth operations.

Job Responsibilities

Responsibilities will reflect requirements of the NHS Contracts, and as such may be subject to change These responsibilities are crucial for ensuring the efficient and effective delivery of CDS, ultimately contributing to improved patient outcomes and satisfaction.

Patient Consultations : Conducting face to face and / or remote appointments. Assessment and Diagnosis - Conducting thorough assessments and making accurate diagnoses based on clinical presentation, history taking, physical examination, and relevant investigations. Treatment and Management - Developing and implementing appropriate treatment plans, recommendations for prescriptions, providing advice on self-care and lifestyle modifications, signposting and arranging referrals to other services when necessary.

Patient Triage : Participating in patient triage processes to prioritise and handle appointments based on clinical urgency and need. GP Education : Conducting or participating in webinars aimed at Westminster referrers, with the purpose of improving management of appropriate conditions at primary care level and improving quality of information within referrals. Patient Education and Empowerment : Providing patients with clear information about their health conditions, treatment options, and self-management strategies to empower them to take an active role in their healthcare.

Joint Clinics : Participating in clinics with a Consultant Dermatologist dealing with advanced clinical cases - learning / accreditation purposes.

Clinical Team Meetings : Participate (and occasionally, Chair) quarterly journal clubs and meetings aimed at clinicians with CDS, for the purpose of sharing learning.

Clinical decision making : Making rapid and informed clinical decisions, often in a fast-paced environment, to ensure patient safety and well-being.

Collaboration with healthcare team : Working collaboratively with members of other services, such as ANPs, nurses, FCPs, pharmacists, and administrative staff, to provide coordinated and holistic patient care.

Documentation and Record Keeping : Ensuring accurate and timely documentation of patient encounters, including clinical notes, investigations, treatment plans, and referrals, in accordance with relevant guidelines and regulations.

Quality Improvement and Clinical Governance : Engaging in continuous quality improvement activities, participating in clinical audits, maintain professional logbooks, and adhering to clinical governance frameworks to maintain high standards of care and patient safety.

Professional Development : Pursuing opportunities for professional development, such as attending relevant training sessions, courses, and conferences, to enhance clinical skills and keep abreast of best practices in primary care.

Adherence to Policies and Protocols : Following established policies, protocols, and clinical guidelines to ensure consistency and standardisation of care delivery within the services.

Patient Advocacy : Advocating for patients' needs and preferences, ensuring they receive respectful, compassionate, and patient-centred care at all times.

Ethical and Legal Compliance : Adhering to ethical principles and legal requirements governing medical practice, including patient confidentiality, consent, and data protection regulations.
