Job overview

An exciting opportunity has arisen to join a busy and passionate CMHT within the centre of Swansea as a CPN. The role will consist of working as part of a multidisciplinary team in undertaking the role of the Care Coordinator in the delivery of a high quality community mental health service under the framework of the Mental Health Measure (2010) Wales, whilst maintaining the Recovery ethos at the forefront of nursing care.

Main duties of the job

To assess, implement and evaluate agreed care and treatment plans for each individual on the caseload which has previously been agreed with the service user / carers. To work autonomously but under the indirect supervision of a Band 7 registered nurse / team manager and in conjunction with other health colleagues in delivery of a supportive service for those people under the care of EIP

If you want excellent career and training opportunities while living on the doorstep of some of Europe's most spectacular scenery, with all the benefits of a thriving and cosmopolitan city - look no further.

Detailed job description and main responsibilities
