We are looking for an experienced, talented, and compassionate Hospice Social Worker for a full-time position.

What will you be doing :

The Hospice Social Worker will work as a part of a home-based interdisciplinary team providing counseling and targeted interventions for a population of homebound patients and their caregivers who are receiving Palliative or Hospice services through NVNA and Hospice. You will conduct an initial psychosocial assessment of the patient / caregiver contributing to the Agency's plan of care. The Hospice Social Worker strives for optimal patient outcomes with improved functioning and an increased ability to self-manage their bio-psych-social challenges. To provide medical services to Hospice patients and / or family members reflective of the philosophy, mission, and standards established by the Agency.

What we’re looking for in you :

• Current MA license in good standing
• Bachelor's degree in counseling or closely related field
• Master of social work from an accredited program (MSW)
• 3 years + experience of social work experience
• Hospice work a plus
• Previous community health and medical health experience preferred
• In-depth knowledge of social service skills and resources
• Good communication skills, both verbal and written
• Ability to harmoniously interact with patients / clients, families, and colleagues
• Handle pressure in various work environments and work in the field
• Ability to work independently with minimum supervision
• Excellent organizational, flexibility, and time management skills
• Keeping up to date with education, policies, and procedures
• Ability to cover the full South Shore field and in the future focus on one central area
• Flexibility for on-call holiday and weekend rotation
