An exciting opportunity has arisen for an enthusiastic and motivated first level registered nurse to join Fife’s Community Diabetes Specialist Nurse Service at 1.0 WTE.
Fife Community Diabetes Specialist Nurse Service operates Monday to Friday between the hours 08 : 30 and 5pm with a 60 min lunch break. The service act’s as an interface between Primary and Secondary Care for patients with complex diabetes. We aim to prevent unnecessary hospital admissions and support patients towards self management of their diabetes if possible.
We are committed to continually improving the patient journey, through collaborative working with other Health and Social Care and multi-agency teams. We work closely with the Secondary Care Diabetes Service, including the In-patient DSN’s, to provide early supported discharge. We provide patient and professional education sessions; advice and support to other professionals including GP Practices, Community Nursing Teams, Care Homes and Community Hospitals.
You will demonstrate a passion for diabetes care and have excellent communication skills. You will be able to work autonomously and as part of a small team. You will have evidence of recent continual professional development at degree level minimum. Prior achievement of a recognised diabetes specialty module is essential. You will be willing to complete post-graduate level study in diabetes in line with paper 8 transforming the role of CNS.
This post involves the use of many IT systems. Proficiency with Microsoft packages including EXCEL is essential.
You will have the ability to reliably and efficiently travel to multiple sites across Fife including patient’s homes.
To be considered as AFC Band 6 applicant the following criteria must be met :
- Minimum 2 years 1st level nursing experience and current registration with the NMC.
- Minimum 1 year’s relevant direct clinical experience in the assessment and management of diabetes within relevant diabetes specialist area or General Practice or Community Nursing.
- Independent Non-medical Prescribing Qualification.
- Recognised Post-registration Diabetes Module or ready to undertake MSc level diabetes module ASAP.
- Demonstrate a good pharmacological knowledge of the various diabetes medications and treatments.
If you meet the above criteria, in return our service offers regular supervision and supports training opportunities. You will be allocated a mentor and supported to develop your existing diabetes knowledge and skills. You will be fully supported with initiating or adjusting insulin and achievement of national TREND / WAND CSN competencies and diabetes post-graduate modules.
ANNEX 21 AFC band 5-6 Trainee :
An independent Non-Medical prescribing qualification and Diabetes post registration module qualification is essential to the role.
Applicants who do not meet the essential criteria to be considered as AFC band 6 but can demonstrate relevant clinical experience or genuine passion for diabetes care and a willingness to undertake all necessary training may still be considered under annex 21 as a band 5 who will progress to band 6 after achievement of TREND / WAND diabetes CNS competencies and NMP qualification.
