uk Phoenix healthcareKeele, Staffordshire, United KingdomJob descriptionJob SummaryWe are seeking dedicated and experienced Mental Health support workers with extensive challenging behaviour and self harm experience to join our team in providing exceptional care to a young male with in his own home. The role involves assisting with daily activities and attending Keel University.Hours of Workx2 10 Hour Shifts per week.Responsibilities- Support patients with personal care tasks such as washing, dressing, and feeding- Assisting with clinical tasks as designated in the care plan- Assist in the implementation of care plans tailored to individual patient needs- Monitor and report changes in patient health status to the healthcare team- Help patients with mobility, ensuring manual handling training is followed- Maintain accurate and up-to-date records of patient activities and progress-Working as part of a dedicated client care teamSkills- Experience in Mental Health- Experience in challenging Behaviour- Self harm and suicide awareness training essential- Breakaway knowledge- Proficiency in IT systems for record-keeping purposes- Excellent communication skills to interact effectively with patients and healthcare professionals- Ability to drive is advantageous due to shift times- Familiarity with creating and implementing care plans for patientswhat we offer
