Positive Futures are looking to recruit a Young Person Resilience Worker, Physical and Mental Health Lead. This role will play an integral role within our Gateshead Young Persons Service to engage young people in a range of interventions, using a person-centred approach, and empowering our young service users to reduce risk, increase resilience and lead safe, happy, and healthy lives.
You’ll have to record information about what you do, so enjoying a bit of paperwork and attention to detail is important. But most of the time you’ll be enjoying working with people from all backgrounds, making a difference to their lives. No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.
Where : Gateshead
Full Time Hours : 37.5 per week
Full Time Salary : £27,449.53 - £31,529.41 dependent on experience (based in full time working hours, pro rata if part time)
Contract : Permanent
Responsibilities
About The Role :
- Engaging young people in a pragmatic & meaningful way, which empowers them to access ongoing support.
- Working closely with our partners within Mental and Physical health services.
- Work holistically to assess need & develop a collaborative package of support for young people, including brief, harm reduction, 1 : 1, group, and clinical interventions
- Carry out comprehensive service user assessments and develop individual care plans that clearly identify how achieving each goal will enable progression in addressing substance use and related outcomes.
- Work closely with your service’s Safeguarding Lead, to ensure all issues and concerns are escalated and dealt with quickly and effectively.
- Carry out risk assessments and risk management plans.
- Being responsive to the needs of young people, ensuring interventions are accessible (. facilitating home visits, working with colleges, universities, etc).
