We are currently looking for a Complex Care Manager to join our Bespoke and Team Brain Injury Support teams, part of the Acuity Care Group. As Complex Care Manager, you will be responsible for managing support packages with varying requirements. This is a field-based role involving regular travel across the area.

Responsibilities :

• Manage and coordinate complex care packages.
• Conduct assessments and develop care plans.
• Ensure compliance with regulatory standards.
• Provide support and guidance to team members.
• Maintain effective communication with clients and stakeholders.
