This long-term contract offers a unique opportunity to make a significant difference to the lives of residents in care homes across Barnet. As part of a proactive and dynamic multi-disciplinary team, you will provide both reactive and proactive face-to-face and virtual clinical support to residential and nursing home residents. This role is key in the delivery of high-quality, person-centred care, supporting complex healthcare needs and ensuring seamless, integrated care planning across services.
