This is a fantastic opportunity to work within a supportive and well-established NHS community team. Key Responsibilities : Manage a defined caseload under the supervision of a Band 6 Nurse, including specialist assessments, care planning, and evaluation of treatment plans in line with Care Programme Approach (CPA) and Single Assessment Process (SAP).Contribute to the setting and monitoring of quality standards, in line with clinical guidance and Trust policies.Administer medication in accordance with NMC guidelines and Trust procedures.Assess individual care needs and deliver tailored interventions based on agreed care plans and pathway guidance, including both pharmacological and psychologically informed approaches.Complete and contribute to comprehensive risk assessments and risk management plans, promoting positive risk-taking to enhance quality of life and independence.Maintain accurate documentation and provide verbal and written reports on patient progress and conditions.Use agreed outcome measures to assess and review the effectiveness of interventions as part of the care planning process.Support train...
