About The Role

Location : This role will be based in the South Eastern Trust

Contract : Permanent

As a Dementia Adviser, you will have the rewarding opportunity to provide support, information, and guidance to people with dementia and their carers; helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives. This is a real opportunity to support all those affected by Dementia in the South Eastern Trust area.

CrISP aims to improve the knowledge, skills and understanding of those caring for a person with dementia by providing effective support and up to date, relevant and evidence based information. CrISP sessions are delivered face to face or virtually.

The service is unique to each service user as it is based upon their personal circumstances and support needs. You will offer support to your clients in a variety of ways, whether in the client's own home, or at other locations in the community, face to face, by phone, letter, or email. You will also assist service users to access other services, providing signposting and referrals.

Your role as a Dementia Adviser will include :

  • Supporting people affected by dementia, families and carers to access vital services. This can be done by providing individual support to running groups and providing to information.
  • Receiving referrals into the CrISP service, organise sessions and deliver programmes
  • Improving people’s sense of well-being, enabling them to have more control over their lives.
  • Assisting with the identification of needs, providing information and access to relevant services.
  • Promoting our local and national services and building relationships with a range of health and social care professionals.
  • We work well with our partners in the area and as a Dementia Adviser you will build a supportive and meaningful relationships with all those working to empower those impacted by Dementia.

About you

  • Ability and interest in supporting people to maintain their independence, improve their sense of well-being, and help them take more control of their lives.
  • Understanding of dementia and the needs of those living with dementia and their carers would be an advantage but if you have had experience supporting with other diagnosis or support requirements this would be taken into consideration.
  • Experience with communicating with a wide range of people, adapting your approach to ensure understanding at all levels.
  • Able to network effectively and collaborating with other professionals to achieve a positive outcome for people.
  • Ability to work as part of a dynamic team of colleagues and partners in a creative and innovative way.
  • Good IT skills, experience working with databases and virtual meeting software (Teams / Zoom).
  • Confident presentation skills through face to face and virtual delivery.
  • Excellent organisational skills as the role has administration responsibilities.
  • The ability to travel across local area independently when required.
  • Closing date : 14th March
