Job description
The Adult & Autism Team are looking for an experienced Social Worker to join them to help promote choice, control and independence for the service user, ensuring that that are at the centre of everything that you do, and that their best interests are met.
Responsibilities include, but are not limited to :
- Working with individuals and their families / carers to complete Care Act assessments, care and support plans, reviews, and carers assessments.
- Safeguarding service users.
- Ensuring effective implementations of department policies and procedures are in line with statutory responsibilities.
- Working with partner agencies.
- Developing effective relationships and communications with colleagues, service users and all other professionals in order to deliver appropriate interventions.
- Working as part of a hard-working team effectively.
