Role : Occupational Therapist - Peripatetic
Location : Remote / National various locations Staffordshire.
Hours : Full time 37.5 hours
JOB SUMMARY Based from home the post holder will have responsibility for the assessment of people with a disability and provide the appropriate recommendations to assist in the statutory obligations under current legislation this may include advice, provision of equipment, adaptations to the home and / or referral to other agencies and possibly assessment for Blue Badge.
The successful candidate will be experienced working in Adult Social Care, or experience / knowledge of the Disabled Facilities grant legislation / process including completion of simple and complex assessments and plan approvals for Disabled Facilities grants.
The Occupational Therapist will be responsible for the management of a caseload which may include the more vulnerable service users or those with complex needs.
The Occupational Therapist will work on specific contracts with Local Authorities, which have been outsourced to NRS Healthcare and will be prepared to undertake reasonable travel across the country dependent upon the needs of the contract and the business.
KEY RESPONSIBILITIES The post holder will :
Complete face to face assessments to assess both adults and children’s needs in accordance with current legislation and anti-discriminatory practice, promoting self-directed support and ensuring demonstrable outcomes.
To undertake / coordinate risk assessments (manual handling) as and when required and to be skilled in carrying out specialist assessments and interventions including risk assessments, Single Handed Care assessments, Reablement assessments, Paediatric assessment, and assessments in relation to the provision of equipment and adaptations under the Chronically Sick and Disabled Persons Act 1970 and Care Act 2014.
Discuss with people the outcomes of their assessment, identifying the choices and options to meet their needs and to maximise their independence, always ensuring that value for money is achieved.
Plan an agreed programme of therapeutic intervention with agreed goals and to ensure that the plan is carried through.
Work in conjunction with service users, carers, other professionals, and agencies, to coordinate and implement agreed service provision.
Provide equipment to assist with daily living and to be proficient in the instruction and demonstration of equipment to aid daily living to service user and carers to enable them to achieve maximum functional independence.
Assist people to develop their daily living skills to enable them to attain maximum independence and evaluate the outcomes of any interventions and advise service user, carer, and other professionals on disability issues.
Organise any necessary property adaptations with the appropriate involvement of other agencies and to recommend, design and monitor the process for the provision of adaptations.
Maintain an up-to-date knowledge of medical conditions and treatment, disability issues, legislation and departmental policies and practices and to liaise with colleagues and manager regarding modifications in practice that may be required as a result.
Provide day to day support to OT Assistants / carers.
Maintain detailed and accurate case records and provide written reports related to people’s services and / or carers needs when appropriate.
Attend and participate in case discussions and conferences as required.
Keep up to date with legislative changes and best practice in the field of Occupational Therapy.
Raise awareness of the needs of people with a disability including sensory impairment by networking with other professional colleagues and other groups as required.
Participate in regular supervision and consultation with the Line Manager and to ensure that the Line Manager is made aware of all issues of major importance or concern (in the absence of the Line Manager this should be another relevant manager).
To manage assigned work in accordance with NRS policies and procedures contributing to the overall performance of NRS Healthcare including leading on predetermined projects and taking cases of a more complex nature, offering support and supervision to less experienced staff as appropriate.
Work, within the professional ethics and disciplines of appropriate professional bodies.
Be able to work autonomously and manage their own caseloads to a high standard of expertise.
Any other duties that may be required that are commensurate with the grading of the post.
This position will be subject to an enhanced or basic Disclosures and Barring Service Check.
