Job Description
As a Part Time Home Care Consultant, you will play a pivotal role in ensuring the seamless introduction of home care services to new clients. You will be responsible for meeting new clients and discussing their needs, coordinating and organising care plans, liaising with healthcare professionals, and ensuring that our clients receive the highest standard of care tailored to their specific needs.
Key Responsibilities :
- Discuss care needs with clients enquiring abiut our services and promote Home Instead positively to them
- Develop and implement personalised home care plans in collaboration with clients, their families, and healthcare professionals.
- Coordinate and schedule home care visits, ensuring timely and efficient service delivery.
- Liaise with a multidisciplinary team of healthcare professionals to provide comprehensive care solutions.
- Monitor and assess the ongoing needs of clients, adjusting care plans as necessary.
- Act as a point of contact for clients and their families, addressing inquiries and concerns with empathy and professionalism.
- Maintain accurate and up-to-date records of client care and progress.
- Collaborate with community resources to enhance the support network available to clients
- Develop networks in the community to increase client referrals.
- Support the Care Manager as requested
- Take part in the On Call rota
Requirements :
Proven experience in care coordination or a related field.Excellent organisational and communication skills.Compassionate and client-focused approachAbility to succcesfully convert client enquiries .Ability to work collaboratively in a multidisciplinary team.Knowledge of relevant regulations and best practices in home care.Benefits :

Salary : £17-18,000 p.a. (dependent on experience)
