We are looking for a passionate and dedicated Community Matron to join our friendly and dynamic team based at Kent Science Park in Sittingbourne.
Working closely with our Specialist Nurses, Community Nursing teamsand MDT Co-ordinatorsin the Sittingbourne locality,you’llwork as a highly skilled case managerdeliveringhigh quality care to patients. Essential to the role will be supporting patients in improving their quality of life, promoting excellence of health and independence within the community, preventing inappropriate hospital admissions and facilitating early discharge, as well as providingeducation, support with self-management and health promotion.
You’ll care for patientsand their families in the way thatyou believe is best, delivering our high service standards, sharing bestpractice, and actively improving the way we work.

The Ideal Candidate
You will be a Registered Nurse with a current NMC registration with a minimum of 3 years’ experience. You'll have experience in a variety of settings including hospital and community and will have evidence of a NMP qualification or will be willing to work towards. You will have strong communication and organisational skills and a flexible approach that will ensure you deliver positive experiences to patients at every stage of their healthcare journey.
You will be a part of quality improvements, research and development activity. Here we recognise and celebrate clinical excellence and dedication.
You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
