Band 6 Occupational Therapist - Discharge to Assess Team - Basildon, earning up to £26 per hour, ongoing. We have a full-time opportunity for you to join our Discharge to Assess Team in Basildon. As a Band 6 Senior Occupational Therapist, your expertise will make a difference daily. This position has no fixed end date, offering an immersive environment. You will collaborate with a Senior Physiotherapist, CHC / AACC Nurses, Domiciliary Care staff, and care home staff.

Responsibilities :

• Facilitate smooth hospital discharges, ensuring patients receive appropriate support at home.
• Collaborate with a multidisciplinary team to develop comprehensive care plans.
• Assess patient needs and capabilities; develop tailored rehabilitation plans.
• Guide community staff, care home workers, and families.
• Evaluate and update treatment plans to maximize rehabilitation potential.
