Social Worker - Adult Social Care - Locality

Ackerman Pierce is currently supporting a leading local authority within Bromley to recruit for a Social worker within the Adult Locality Team on a permanent basis paying up to £39,709 - £47,509 per annum plus £1,000 Golden Hello and £1,500 Annual Retention Allowance Main Purpose : To promote and maintain independence for vulnerable adults using a strengths-based approach. To protect adults at risk by the use of procedures to safeguard adults from abuse. To assess the needs of individuals and their carers in line with Care Act (2014) criteria. Together with the service users and carers, formulate and implement person-centred care and support plans Monitor, review and evaluate services in order to ensure that they are provided in accordance with eligible needs. Responsibilities include : Ensuring the ethos of wellbeing is adhered to when working with vulnerable adults and their carers. Participate in the operation of duty and respond accordingly to need, which will involve Safeguarding enquiries and Best Interest Assessment (BIA) when required. Provide an assessment service to individuals, their carers and other agencies in accordance with Care Act (2014) criteria. Initiate and co-ordinate care and support plans for and with individual service users and support plans for carers, in conjunction with professional colleagues and representatives of voluntary and private agencies.
