Job descriptionYou must have experience within Learning disabilities, community DOLs, safeguarding Conduct comprehensive assessments and develop personalized care plans for individuals and families in the community. Provide direct intervention and support services to address issues like mental health, substance abuse, and homelessness. Collaborate with multi-disciplinary teams and community partners, maintain accurate documentation, and evaluate interventions to ensure effective client outcomes.
