Job description

Job summary

Paramedicswork autonomously within the community using their enhanced clinical assessmentand treatment skills, to provide first point of contact for patients presentingwith undifferentiated, undiagnosed problems relating to minor illness orinjury, abdominal pains, chest pains and headaches. They are healthprofessionals who have the capability to make sound judgements in the absenceof full information and to manage varying degrees of risk when there arecomplex, competing or ambiguous information or uncertainty.

Main duties of the job

1. work as part of a MDT within the PCN;

2. assess and triage patients, including sameday triage, and as appropriate provide definitive treatment (includingprescribing medications following policy, patient group directives, NICE(national clinical guidance and local care pathways) or make necessaryreferrals to other members of the Primary care team

3. advise patients on general healthcare andpromote self-management where appropriate, including signposting patients to prescribingservice, and where appropriate, other community or voluntary services.

4. be ableto :

a. perform specialist health checks and reviews within your scope of practice andin line with local and national guidance.

b. perform and interpret ECGs, alongside other results asappropriate

c. perform investigatory procedures asrequired

d. undertake the collection of pathologicalspecimens including intravenous blood samples, swabs and other samples withinyour scope of practice, and within line of local and national guidance.

e. perform investigatory procedures needed bypatients and those requested by GPs and the wider PCN

5. support the delivery of anticipatory careplans and lead certain community services ( monitoring blood pressure anddiabetes risk of elderly patients living in sheltered housing)

6. provide an alternative model to urgent andsame day home visits for the PCN and undertake clinical audits

About us

GP practices have been working together in our area for years and formed BridgedalePrimary Care Network (PCN).Bridegale Primary Care Network is made up of GP registered patient lists for Roslea Surgery, Riverside Medical Centre and the Ryan Medical Centre. We serve a community of around 30,000 people. We are small enough to provide the personal care valued by our people and large enough to have impact and economies of scale through better collaboration.

As a Primary Care Network we are working hard to deliver on the national NHS England network contract that commenced in July 2019 and continues to March 2024.

This includes the following areas of focus :

anticipatory care

cardiovascular disease prevention and diagnosis

enhancing health in care homes

early diagnosis of cancer

tackling neighbourhood inequalities

structured medication reviews

social prescribing reviews

So as a primary care network ,with its member practices working ever closer together, it is important to establish shared core values which not only enables us to adapt, but to do so in the way that we all feel is right for our patients and staff. prescribe, issue and review medications asappropriate following policy, patient group directives, NICE (national) andlocal clinical guidelines and local care pathways. This will be done inaccordance with evidence-based practice and national and practice protocols,and within scope of practice

work with patients in order to supportcompliance with and adherence to prescribed treatments

provide information and advice on prescribedor over-the-counter medication on medication regimens, side-effects andinteractions

assess, diagnose, plan, implement andevaluate treatment / interventions and care for patients presenting with anundifferentiated diagnosis, and patients with complex needs

clinically examine and assess patient needsfrom a physiological and psychological perspective, and plan clinical careaccordingly

prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care

diagnose and manage both acute and chronicconditions, integrating both drug- and non-drug-based treatment methods into amanagement plan

support patients to adopt health promotionstrategies that promote healthy lifestyles, and apply principles of self-care

Person Specification

Experience

Essential

  • experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans
  • working knowledge of Microsoft and GP practice and prescribing data monitoring systems
  • ability to write comprehensive clinical notes, implement and evaluate care plans
  • be aware of data protection (GDPR) and confidentiality issues particularly within a PCN
  • ability to evidence a sound understanding of the NHS principles and values
  • ability to analyse and interpret complex / often incomplete information, preempt and evaluate issues, and recommend and appropriate course of action to address the issues

  • Qualifications

    Essential

  • BSc in a training programme approved by the College of Paramedics
  • Educated to degree / diploma level in Paramedicine or equivalent experience
  • Health & Care Professions Council (HCPC) registration
  • Has completed two-year Consolidation of Learning period as a newly qualified paramedic and has a further three years experience as an AFC Band 6 (or equivalent) paramedic.
  • In addition, is working towards developing masters level or equivalent capability in paramedic areas of practice and within six months of commencement of the role, have completed and been signed off formally within clinical pillar competencies of the paramedic FCP / AP roadmap to practice
