As a Primary Care Network we are working hard to deliver on the national NHS England network contract that commenced in July 2019 and continues to March 2024.
This includes the following areas of focus :
anticipatory care
cardiovascular disease prevention and diagnosis
enhancing health in care homes

social prescribing reviews
So as a primary care network ,with its member practices working ever closer together, it is important to establish shared core values which not only enables us to adapt, but to do so in the way that we all feel is right for our patients and staff.
