Job summary

We have a fantastic opportunity for a Nursing Assistant to join our Myton at Home team. The Myton at Home service provides expert and compassionate care to patients in their own homes, at a crucial time in their lives.

This role involves home visits within the Warwickshire area. You will be based at our Warwick Hospice but required to work at our Rugby and Coventry Hospices on occasion.

The shifts are 8am - 8pm and the role includes weekend working.

Main duties of the job

Our Myton at Home Nursing Assistants :

  • Support Registered Nurses to deliver holistic nursing care
  • Communicate sensitively with patients, families and caregivers
  • Assess and respond to patients needs
  • Maintain patient records
  • Assist patients with personal and emotional care

About you

We would love to hear from you if you have :

  • Experience of caring for patients and families with life-limiting / life-threatening conditions
  • Understanding of safeguarding
  • Compassion
  • Communication skills
  • NVQ level 3 (or equivalent), or willingness to work towards this (essential)
  • UK driving licence
  • The right to work in the UK (we cannot support with sponsorship)
  • Other roles you may have experience of could include : Healthcare Assistant, HCA, Health Care Assistant.

    We will carry out an enhanced DBS check including the childrens and adults barred list because you will be working in regulated activity with patients and their families. We will cover the cost of the check.

    Job description

    Job responsibilities

    To support the nursing team in the delivery of nursing care to the patients and their family / caregivers, in their own home, providing evidence based, individualised end of life care. To support the Primary Healthcare teams in the delivery of its services, in line with the District nurses care plan, whilst adhering to statutory regulations and Hospice policies.

  • To work under the direct and indirect supervision of the Hospice at Home Lead, in the delivery of quality holistic nursing care for patients in their own homes.
  • Respond to the specific needs of palliative care patients and their significant others, in line with agreed care plans.
  • Recognise and report any changes in patients condition and where appropriate provide care and support under the direction of a Registered Nurse.
  • Communicate effectively and sensitively to patients, their significant others and other professionals, referring questions and concerns to the appropriate personnel.
  • Assess and reassess patients capabilities as needs change regarding assistance with the activities of daily living and emotional care including;
  • Personal care including hair, nails, shaving.
  • Mouth care.
  • Toileting needs, continence issues / care.
  • Drinking and eating.
  • Mobility, assisting patients and utilising safe moving and handling techniques / equipment to help maintain skin integrity.
  • Comfort measures and reassuring presence.
  • Provide emotional support to patients and those that are important to them, including post bereavement support.
  • Ensures safe and responsible use of equipment identifying and reporting any malfunction, taking action to maintain safety as directed.
  • To provide practical support to the family in terms of guidance and advice around safe delivery of personal care when appropriate.
  • Where appropriate assist with last offices and support other nursing staff with the care of the deceased.
  • Help promote and maintain an environment that is safe and conducive to meeting the needs of patients, their relatives and friends.
  • Promote and maintain dignity and privacy at all times
  • To take into account, and meet, the physical, emotional, spiritual, and social needs of the patient, their relatives and friends.
  • Actively contributes and promotes a team approach to care based on mutual support and respect

  • To create and maintain accurate contemporaneous patient records, ensuring compliance with Myton Hospice policy and procedures
  • Take all measures to ensure the safety and welfare of all patients and staff, having due regard to the Health and Safety at Work Act. Actively contribute to developing the service to enhance the care and support available to patients and families.
  • Be responsible for own continued development
  • Manage clinical duties and resources effectively to ensure the smooth running of the service, working alone for periods adhering to The Myton Hospices Lone Worker Policy.
  • Ensure that their transport is road-worthy and fit for use as per policy.

    Experience

    Essential

  • Two years' experience of hospice, hospital or community nursing.
  • Experience of multidisciplinary team working.
  • Experience working in a palliative care setting.
  • Experience of caring for patients and families with life limiting conditions / life threatening conditions.
