Job summary

We are looking torecruit an enthusiastic, passionate and driven Clinical Pharmacist to join ourpharmacy team in Didcot Primary Care Network. You will work as part of thepharmacy team alongside other practice staff to ensure the timely, safe andaccurate prescribing of medication within the Network.

We are recruiting anadditional clinical pharmacist to initially cover the maternity leave of anexisting pharmacist, and expand our current team of clinical pharmacists andpharmacy technicians.

The role ispredominantly patient-facing, split between fielding medication enquiries,managing cardiovascular disease and providing input into local care homes. Onesession per week is set aside for non-patient facing quality improvement work.You will work as part of a multi-disciplinary team to develop and run processesfor management of medicines and safer prescribing across the three practices inDidcot.

This role is bestsuited to someone who has completed or is nearing completion of their CPPEpathway. Support in undertaking the independent prescribing qualification willbe available if not yet completed.

Main duties of the job

  • Clinicalmedication reviews for patients with single or multiple long term conditionswhere medicines optimisation is required.
  • Toreconcile medicines following discharge from hospitals or intermediate care andworking with patients and community pharmacists to ensure patients receive themedicines they need post-discharge and to reduce risk of readmission.
  • Contributeto the provision of out of hours / on call / extended services for the practice andthe patients.
  • Tomanage repeat prescription requests, resolving queries where possible withinscope of practice.
  • Implementchanges to medicines in line with MHRA alerts, product withdrawal or shortageand other local or national guidance.
  • Tomaintain full and complete records of all patient contacts using appropriateclinical templates and coding.
  • Provideindependent information to patients with regard to medicines and prescribingchanges, initiating further support from other healthcare professionals whereappropriate.
  • Participatein review and setting of policies relating to medicines management andprescribing and help ensure practices prescribe in accordance with localguidelines and formulary.
  • Toidentify areas of clinical risk at network level and make recommendations tosupport the introduction of new working practices that will optimise thequality of prescribing.
  • Tomonitor network practices prescribing against the local health economy andmake recommendations for GPs or amend within scope of practice, based onfindings

About us

Didcot PCN comprisesof 3 GP Practices in the expanding town of Didcot which serves a population ofaround 45,000. The 3 practices have a close working relationship which has beenstrengthened by working together on the Covid vaccination programme. You willjoin our current pharmacy team of 3 clinical pharmacists and a pharmacytechnician in supporting and working across the 3 practices.

Tomanage repeat prescription requests, resolving queries where possible withinscope of practice. To ensure patients have appropriate monitoring tests inplace when required.

Implementchanges to medicines in line with MHRA alerts, product withdrawal or shortageand other local or national guidance.

Tomaintain full and complete records of all patient contacts using appropriateclinical templates and coding To undertake a proactive role in audit andquality improvement implementing recommendations where appropriate.

Provideindependent information to patients with regard to medicines and prescribingchanges, initiating further support from other healthcare professionals whereappropriate.

Participatein review and setting of policies relating to medicines management andprescribing and help ensure practices prescribe in accordance with localguidelines and formulary.

The post-holder willparticipate in any training programme implemented by the PCN as part of thisemployment, such training to include :

Trainingmay need to be undertaken outside of normal practice hours, and off site.

Person Specification

Experience

Essential

  • Minimum of 2 years post-qualification experience.
  • An appreciation of the nature of GPs and general practices
  • An appreciation of the nature of primary care prescribing concepts of rational prescribing and strategies for improving prescribing.
  • Desirable

  • Specialist knowledge acquired through postgraduate diploma level or equivalent training / experience
  • Independent prescriber or working towards / intent of gaining an independent prescribing qualification
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare
  • Qualifications

    Essential

  • Mandatory registration with General Pharmaceutical Council (GPhC)
  • Masters degree in pharmacy (MPharm) (or equivalent)
  • Desirable

  • Membership of the Royal Pharmaceutical Society
  • A member of or working towards Faculty membership of the Royal Pharmaceutical Society
