An exciting job role has recently become available at the Middlesbrough Metropolitan of Lewisham. They are looking for a new member for their Hospital Discharge Team.
X4 workers required for a 12 month period 2 years post qualifying experience required Request to cover case work / reviews. This will involve the ability to undertake face to face visits x5 days a week with x2 days a week working from an office setting. Learning Disability Team – adults in transition and over 18 with a learning disability
