and main responsibilities

The overall purpose of this post is to support the delivery of the End of Life Care Strategy within LNWH. The post holder will be involved in the delivery of nursing care for patients in the last phase of life with the level of knowledge, skill and competence beyond that of a traditional healthcare assistant. You will support and contribute to education and training of registered nurses, healthcare assistants and other members of the multi-disciplinary team (MDT), with regards to the specific needs of patients and their families / carers at the last phase of life.

Working under indirect supervision, as part of the nursing team, you will have responsibility for delivering nursing care to patients within a delegated caseload.

You will participate in multidisciplinary working and the development, implementation and evaluation of the delivery of health care services.

Education / Qualifications

Essential criteria

  • FDSc Assistant Practitioner Foundation degree

Desirable criteria

  • NVQ Level 3 Health Award
  • Knowledge and Experience

    Essential criteria

  • Experience working in the acute Healthcare setting
  • minimum of 3 years experience as a healthcare assistant
  • Demonstrates leadership and teaching ability
  • Ability to work on own initiative
  • Experience of team working
  • Experience of training others
  • Experience of working with patients in last days of life
  • Skills, Abilities and attributes
