ThePCN Advanced Paramedic Practitioner role at Farnham Integrated Care Services(FICS), Farnham Centre for Health, will be a registered Paramedic with postgraduate qualifications, including the Independent and Supplementary Prescriberqualification and HCPC registration, acting within their professionalboundaries.
Thisrole will work as part of a multi-disciplinary team of AHPs / GPs / HCAs etc,providing care for the presenting patients within our Urgent Same Day CareService at FICS. An established integrated service supporting the GP Practicesof Farnham.
Frominitial history taking, clinical assessment, diagnosis, treatment andevaluation of their care. They will demonstrate safe, clinical decision-makingand expert care for patients within the service
Main duties of the job
The PCN Advanced Paramedic Practitioner is expected to attend to patients within the defined list of conditions, including assessment, diagnosis and treatment to deliver quality patient services in conjunction with the daily duty doctor.
The PCN APP will work collaboratively with the multi-disciplinary team to meet the needs ofpatients, supporting the delivery as per policy and procedures. There is an expectation that trainingacross the service is maintained to an expected level with use of appropriate equipment andfacilities as directed by the post holder who is an autonomous allied healthcare professional whowill be part of PCN support to practices while working at FICS.
The post holder will use advanced clinical skills to provide education to service users, promoting selfcare and empowering them to make informed choices about their treatment.
The PCN APP will actively support integrated ways of working to support standardization forefficiency purposes.

Alongside themultidisciplinary team, to provide a first point of contact for patientspresenting with undifferentiated, undiagnosed problems. Making use ofskills in history taking, physical examination, problem-solving andclinical decision-making to establish a diagnosis and management planTo provide assessment,treatment and diagnosis at point of first contact by attending to patientswithin our Same Day Service at FICS.To make professionallyautonomous decisions for which he / she is accountableTo refer patients to analternative care setting or treat and discharge as appropriateTo instigate necessaryinvasive and non-invasive diagnostic tests or investigations and interpretfindings / reportsGenerate EPS and FP10prescriptions within scope of practice, considering local and nationalguidelines.To ensure that professionalstandards are maintained and within the guidance provided by theDepartment of Health, the Health and Care Professions Council (HCPC) andthe College of Paramedics (COP)To support the development ofexcellent relationships across the PCN to enable collaboration for betterpatient outcomesTo support networkimplementation of agreed service changes and pathways, working closelywith other staff members, the wider PCN and the commissioner to develop,support and deliver local improvement programmes aligned to national andlocal priorities.To contribute to the serviceachieving its quality targets to sustain high standards of patient careand service deliveryTo participate inidentification of community health needs and develop patient / family-centredstrategies to address themTo help develop and set upnew patient services and participate in initiatives to improve existingpatient servicesTo support in the delivery ofenhanced services and other service requirements on behalf of the PCNTo participate in themanagement of patient complaints when requested to do so and participatein the identification of any necessary learning brought about throughclinical incidents and near-miss eventsTo undertake all mandatorytraining and induction programmesTo contribute to and embracethe spectrum of clinical governanceTo attend a formal appraisalwith their manager at least every 12 months.
