Job descriptionConduct assessments under the Care Act 2014 and the Mental Capacity Act 2005, working directly with individuals, families, and professionals. Take charge of Care Planning and offer short-term case management to adults requiring care and support. Participate in Multi-Disciplinarymeetings and manage initial Safeguarding enquiries.
