Job descriptionConduct comprehensive assessments of service users to determine their needs and eligibility for services. Develop, implement, and review care plans in collaboration with service users and their families. Coordinate and manage safe and timely discharges from hospital settings. Work collaboratively with other professionals such as doctors, nurses, and therapists to ensure a holistic approach to care. Advocate for the rights and needs of service users, ensuring they have access to the resources and support they require.
