• Conduct CHC assessments and reviews in line with NHS Framework standards
• Communicate complex, sensitive information with individuals, families, and professionals
• Analyse and interpret specialist conditions, producing detailed reports
• Collaborate with multi-disciplinary teams to ensure the best patient outcomesKey Skills & Experience :
• Registered Nurse with previous CHC assessment experience
• Strong communication and interpersonal skills, handling complex, sensitive cases with empathy
• Skilled in negotiation and influencing, working collaboratively with stakeholders
• Excellent written communication – producing clear, comprehensive reports
• Strong analytical and judgement skills – assessing complex conditions
