Job descriptionConduct comprehensive reviews and reassessments of care packages for adults. Develop and implement care plans tailored to individual needs. Work collaboratively with service users, families, and other professionals to provide holistic support. Maintain accurate and up-to-date records in line with statutory and local requirements. Ensure all actions are compliant with relevant legislation, policies, and procedures. Advocate for the rights and needs of service users. Participate in team meetings and contribute to service development initiatives.
