Job descriptionResponsible for undertaking assessments for children with disabilities and complex needs. Leading plans for children with disabilities and complex needs You will be responsible for the implementation, monitoring, and reviewing of plans within multi-agency meetings relating to the assessed complex needs of children, young people, and their families.
