Job summary

Unplanned Care - Community Staff Nurse (Band 5)

Are you a passionate and caring nurse either qualified or in training looking to work in a dynamic community nursing team?

If so, then we would like to meet you.

A vacancy has arisen within the Unplanned Care Team, which has an innovative approach to nursing care. It involves quick responses to patients in their home to assess and manage the presenting issue, prevent unnecessary hospital admissions, and help to facilitate early hospital discharges.

We are looking for a highly motivated applicant who is interested in meeting the challenge of working in the community with patients in their home setting. As part of the Unplanned care team, you will be working with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients. You'll be working on a rotational basis as part of a seven-day service covering 24-hours-a-day with the support of agile technology to help you deliver timely, high-quality care.

Main duties of the job

To work as part of the community nursing team in unplanned care ensuring high quality health care is delivered to patients in their own home. The post holder will be responsible for their own workload and work in collaboration with the wider Community Partnerships and Primary Care Networks. The post holder will be responsible for participating in the assessment of health needs, delivery of care to the practice population, preventing hospital admissions and facilitating early discharges from hospital. The unplanned service is delivered over a seven-day rota, covering 24-hour shift pattern. Working agilely across the service.

Qualifications

Essential

  • Current NMC Registration

Desirable

  • NMC Registered Nurse
  • V100 prescribing or nursing equivalent
  • Teaching / Assessor in Clinical Practice
  • Special Knowledge / Experience / Competence

    Essential

  • Demonstrate an understanding of the role of the District Nursing Service
  • Ability to use Microsoft Office and Outlook
  • An awareness of current Health and Social Care policies
  • Desirable

  • Previous experience of community health care (District Nursing)
  • Knowledge of SystmOne
  • Understanding / experience of managing patients with long-term health conditions.
