Job summary

The Kennet and Avon Medical Partnership is a large general practice. We are seeking a QualityAssurance Assistant to join our team.

This is a non-clinical, part-time role with flexibility on working days between Monday and Friday.

This is an administrative role which plays a key part in optimising the health of our patients with chronic and life-long conditions as well as contributing to the health of our wider population. You will have responsibility for named medical conditions as well as specific health screening and immunisations which will rotate each year.

Main duties of the job

Your main role will be to :

  • Ensure optimal uptake of health promotion initiatives, such as cardiovascular health screening and immunisations.
  • Contribute to the achievement of The Quality and Outcomes Framework (QOF) by managing call, recall and monitoring processes to support clinician review of our patients with long-term conditions such as kidney disease and heart disease.
  • Summarise contents of paper medical records and add relevant information to the patient's electronic record.
  • Assist with processing clinical correspondence, eg from hospitals, adding relevant information to the patient record and flagging any actions to the appropriate clinician.

Job responsibilities

Duties and Responsibilities :

QOF Administration

Ensure all QOF registers are accurate, completeand up to date

Undertake a comprehensive and effective callsand recalls service for all QOF requirements

Monitor progress towards QOF targets and takeaction when targets are not being achieved in a timely manner

Run searches to identify errors in coding ormissed coding leading to patients not being identified as having a specificcondition, code as appropriate

Regularly review missed income reports toensure all registers are fully populated

EnhancedServices Administration

Undertake a comprehensive and effective callsand recalls service for all enhanced services requirements

Monitor progress towards enhanced servicetargets and income and take action when targets are not being achieved in atimely manner

Ensurethat monthly, quarterly and any other data returns to commissioningorganisations are made on time and are accurate and reflect true levels ofactivity

Wheresuch returns are extracted automatically from S1 by the commissioningorganisation, check for and follow through with the commissioning organisation,any material discrepancies between those extracts and the practices owninternal searches

Clinical Correspondence

Receiveclinical correspondence from a variety of sources

Process,clinically code and save to the patient notes any relevant medical information

Asnecessary, pass any actions arising to appropriate clinicians, admin staff orpass to appropriate clinicians for information

Notes Summarising

Receive paper records for all newly registeredpatients

Following the agreed protocol, review the wholepatient record (paper and electronic version), cull and organise the record,produce an accurate summary of the patients past medical history, code anyrelevant information onto the electronic patient record and remove any pastcoding that doesn't comply with KAMP protocol

Experience

Essential

  • Practical experience of computerised recording systems
  • Desirable

  • Experience of complex administration and record keeping in a primary care setting

  • GCSE Mathematics C or above (or equivalent)
