Job summary

We are looking to recruit enthusiastic and motivated Band 5 Community Public Health Staff Nurses for the 0-19 Service (0-5 Health Visiting) across Sheffield. The post includes working in family homes, Family Hubs, and other community venues.

The Community Public Health Staff Nurse will work as part of a skill mix team to support children, young people and families to achieve best outcomes. The successful applicants will support the delivery of the Healthy Child Programme.

The 0-19 team works in partnership with other agencies to deliver a Universal service and identify and support children and young people with complex needs and safeguarding concerns.

Main duties of the job

  • Working under indirect supervision of a Specialist Community Public Health Nurse, (SCPHN), to deliver the Healthy Child Programme
  • Undertake routine home visits.
  • Monitor and evaluate care
  • Work with the child, family, and specialist services to assess needs and plan specific care.
  • To support the 0-19 Team to meet the safeguarding needs of children and families, working according to safeguarding guidelines as defined by local and national policy.
  • Promote the health of looked after children, including completion of review health assessments.
  • Contribute to the planning, implementation, monitoring and evaluation of therapeutic programmes to empower individuals to make informed choices.

Job responsibilities

MANAGERIAL AND LEADERSHIP

  • Use effective delegation skills within the team.
  • Participate in the induction programmes for new staff
  • Provide supervision to pre and post registered nurses and Community Nursery Nurses
  • SERVICE DELIVERY

  • Deliver the Healthy Child Programme
  • Working under indirect supervision of a Specialist Community Public Health Nurse, (SCPHN), to assess the health needs of children, young people and their families utilising an appropriate assessment framework. Report assessment outcomes to the SCPHN and other agencies as appropriate.
  • Communicate and liaise with members of the broader health team, parents, and carers.
  • Refer to other specialist services and agencies as appropriate. Review children who are referred, where appropriate engage in joint work with other services.
  • With supervision from the SCPHN, identify and prioritise children and families who require a rapid response or would benefit most from the support of the SCPHN or other services. Working with the broader health team contribute to an appropriate plan for each child and family who needs particular support.
  • Undertake routine home visits.
  • Monitor and evaluate care
  • Work with the child, family, and specialist services to assess needs and plan specific care.
  • To support the 0-19 Team to meet the safeguarding needs of children and families, working according to safeguarding guidelines as defined by local and national policy.
  • Attend safeguarding meetings as appropriate and feedback to the Band 6 SCPHN. Providing reports and information to other agencies as appropriate.
  • Promote the health of looked after children, including completion of review health assessments.
  • Contribute to the identification of the public health needs of the locality population and determine the priorities for action. Develop and co-ordinate health promotion and public health strategies to address the identified needs.
  • Contribute to the facilitation of group work with parents, children, and others.
  • Contribute to the planning, implementation, monitoring and evaluation of therapeutic programmes to empower individuals to make informed choices.
  • Use time management skills effectively to prioritise work within agreed time scales.
  • PROFESSIONAL DEVELOPMENT AND RESPONSIBILITIES

  • Maintain clinical competency and accountability in accordance with the NMC Code of Professional Conduct. (Revalidation)
  • Work within the policies and procedures of The Sheffield Childrens Hospital NHS Foundation Trust.
  • Maintain accurate and contemporaneous records in accordance with the record keeping policy.
  • Utilise the electronic record inputting and retrieving data where necessary.
  • Utilise research-based practice and developments.
  • Participate in clinical supervision and performance development reviews (PDR) and take responsibility for achieving development objectives.
  • Accurately record and collect data / statistics as required by the service in accordance with agreed timescales.
  • Undertake and contribute to clinical audit
  • Participate constructively within service meetings.
  • Monitor and report to the Team Leader regarding staff safety / accidents / reportable incidents as identified by the policies and procedures.

    Essential

  • Post-registration experience of working with children and / or adults
  • Evidence of multi-disciplinary team working
  • Desirable

  • Evidence of delivering Public Health interventions
  • Demonstrates the ability to recognise vulnerabilities when working with children, young people and families.
