Hybrid working - at least 2 days in the Croydon office.(12-13 Lennard Road. CR9 2RS)

All applicants MUST have UK driving license and own vehicle for possible on site visits within the borough.

Description

• Assessing and reviewing renablement needs and goals, for people who are discharged from hospital via the Discharge to Assess (D2A) Pathway.
• Assessment of social care needs
• Assessing renaablement needs and goals
• Reviewing needs and goals
• Use Social care IT
